How do I book an appointment?

I’ve just booked my initial appointment, are there any forms I need to fill out in advance?
Yes, please check your email inbox, a link to the forms should have been sent to you immediately upon booking your appointment. Can’t find it? No problem – email hello@flourishyourhealth.com and we will send it to you right away.
Does Flourish accept patients who live outside of the GTA?
If you are unable to visit us in clinic, you can book a virtual or phone appointment. Please email hello@flourishyourhealth.com to arrange or book online.

Where is Flourish Health Clinic?
We are located at 1849 Yonge St Unit 314. We are on the third floor of a medical building on the East side of Yonge St between Merton St and Balliol St.
What are the hours of operation?
Appointments are available Monday-Friday 8am-8pm and Saturday 9am-2pm.
Office hours are Monday & Friday 9-4, Tuesday, Wednesday & Thursday 9-6 and Saturday’s 9:30-2
Is there parking available?
There is paid and free street parking available on Yonge, Merton and Balliol streets and there is a large parking lot located at 1941 Yonge St or 68 Merton St.
Is Flourish accessible via public transit?
Yes we are across the street from Davisville Subway station (across Yonge and 1/2 block south)
Is Flourish an accessible space?
Yes, Flourish is fully accessible space with elevators for wheelchairs and strollers and accessible washrooms. If you have any other accessibility requirements please email hello@flourishyourhealth.com prior to your appointment and we will do our best to accommodate you.
Do your patients still need a primary care MD?
At Flourish we recommend that our patients have a MD as well as ND but we can provide annual physicals and requisition blood work if you are unable to see your GP.
Is Flourish a scent-free office?
For the comfort and safety of our clients and staff, we ask that you please refrain from using fragrance while visiting the clinic (and in general, your hormones will thank you!) Essential oils and plants are accepted. We appreciate you understanding.
Payment and insurance billing questions
What forms of payment does Flourish accept?
In order to allow us to focus on your health during our appointments and to streamline customer service, all invoices will be charged automatically to a credit card (Visa/ Mastercard/AMEX) that is kept encrypted your file within 24-48 hrs of your appointment. We do require a credit card on file at time of booking.
Once in clinic, if you choose to pay with Debit, e-transfer or cheque we will accept that and not charge the card on file.
We also direct bill to most insurance companies. This information can be added to your intake form. Please email hello@flourishyourhealth.com or call 416-551-2170 with any questions about direct billing.
Please note that while we will accept cash, we don’t generally keep change on site.
Do You do direct billing to insurance companies?
Yes, we do direct billing to most insurance companies and you are able to provide your insurance information on your intake form. Please note we cannot direct bill to Green Shield, Blue Cross, Empire Life and a few smaller companies but we can provide you with an insurance receipt.
Rescheduling and cancellation policy
How can I reschedule or cancel my appointment?
You can reschedule or cancel through logging into the Jane app HERE, calling us at 1 (416) 551-2170 or sending an email to hello@flourishyourhealth.com
What is your cancellation policy?
Due to the fact that we work with busy people like you, it is often impossible to fill appointment spaces last minute and we therefore require 48 hours notice for any cancellations in order to avoid being charged in full for appointment fees. You can also send flowers, that usually helps.

How does Flourish strive to protect its patients information?
We 100% abide by the Personal Health Information Protection Act:
The Ontario Government has legislation, the Personal Health Information Protection Act (PHIPA) that came into effect on November 1, 2004. This act has been established to set out rules for the collection and utilization of information that is collected about patients.
Our policy for the collection of Personal Information
We collect personal health information about you. We collect this information either from you directly or from the person acting on your behalf (i.e a parent). The information that we collect may include your name, date of birth, address, health history, records of your visits and the care that you received. Occasionally, when we have received your consent, we receive and collect information about you from other sources.
